Error Rate

How often people make a mistake, per opportunity for an error.

Error rate = errors ÷ opportunities × 100

Why it matters

Errors cost time, trust and sometimes money.

How to use it

  • List the actions where errors can happen.
  • Separate slips from misunderstandings.
  • Prevent errors, instead of only explaining them.

Example

If 6 of 200 transfers go to the wrong person, the error rate is 3%.
